It looks like he sees the door open, then fires. Freshly spawned HW is a bigger threat than a demo piping the ramp room from that position. I've let people go as scout as I figured dodging them conferred a bigger advantage with them not seeing me rather than firing my nailgun and exposing myself. Could be a strategy, why waste rockets on the camping demo far away when you can fire on newly spawned players who are a bigger threat.
2nd floor respawn things tend to be a gray area as that's a hot area of the map. I frankly think there should be more leeway with respect to spamming that respawn. "There are legitimate reasons to spam the spawn as well." That's literally on the server rules.

Yeah, they might be right, maybe.
He might've been trying to make it to RR to take care of that demo, but been held back and stuck in front of spawn by the demo's keepout (you see some of the pipe explosions). Hearing spawn footsteps, he'd know someone was about to drop on his position when he's already at half H/A and somewhat stuck, and decided to defend himself pre-emptively since he'd have a better chance at catching them off guard than the demo. (Or not, and he was just camping, Idk.)
I occasionally "defend" myself from footsteps too if I'm legitimately trying to pass by and I feel it's my only or best option, especially if I know the person who's likely spawning is a genuine threat. I do try to delay until I see the whites of their eyes at least (which is an eternity compared to my usual pre-emptive firing based on sound cues). Or if my team is fighting for 2f or boxroom flag and I or others have just nuked their whole team, and I hear 27 footsteps coming out of spawn to immediately begin respamming the flag, I have no qualms about timing my nade so I catch them just as they exit.
Those still might be against the rules, IDK, but I think it's pretty clear I never intend to get cheap spawn kills, and my intent has more immediate and strategic relation to the game's goal. Idk. | |

It’s a fine line but I believe he was facing the demo hardly shown in the video in the ramp room to your right. You came out and he reflexively engaged but I don’t think it was camping for a cheap kill. | |
